metabolic science in skincarePatients arriving at your clinic are more research-literate than they were five years ago. Many are already asking questions about ingredients in their topical products, often framed through the “clean beauty” trend. For most consumers, “clean beauty” remains a marketing term tied loosely to natural-sounding ingredient lists. For practitioners, it’s an opportunity to elevate the conversation with the evidence base patients haven’t seen, particularly the research linking certain cosmetic ingredients to metabolic and endocrine effects that extend well beyond skin health.

This is a conversation that fits naturally into the consultation flow. A patient asking about retinol stability or whether her moisturiser is “good enough” has already opened the door, and the research supports a more substantive answer than ingredient swapping alone.

Endocrine-disrupting chemicals and adipogenesis

Angelica Amorim Amato, Hailey Brit Wheeler, and Bruce Blumberg (2021) published a comprehensive review on obesity and endocrine-disrupting chemicals (EDCs), introducing the clinical concept of “obesogens” as a specific subset of EDCs. Amato and colleagues (2021) explain that obesogens can inappropriately stimulate adipogenesis, the creation of new fat cells, and promote lipid accumulation in the body.

The mechanism is worth knowing in clinical detail. When certain compounds in conventional cosmetic products are absorbed through the skin and enter systemic circulation, they can act as peroxisome proliferator-activated receptor gamma (PPARγ) activators, signalling preadipocytes to differentiate into mature fat cells (Amato et al., 2021). The simple calories-in-calories-out model patients hear from their gym or weight-loss app doesn’t account for this layer of metabolic chemistry, and explaining it positions you as a clinician who understands how skin-applied products interact with broader physiology.

Parabens and what the research shows

Amato et al. (2021) identify parabens as still widely used preservatives in a broad range of cosmetics and personal care products, and note that human exposure to parabens has been linked to overweight development.

The mechanism Amato and colleagues (2021) describe goes beyond simple metabolic disruption. Paraben exposure has been associated with hypermethylation of a specific gene enhancer in the hypothalamus called the POMC enhancer nPE1, which reduces expression of the POMC (proopiomelanocortin) gene (Amato et al., 2021). Because POMC is fundamentally responsible for regulating appetite and energy balance, its suppression has been linked to hyperphagia, an abnormally increased appetite, and is implicated in driving weight gain (Amato et al., 2021).

For patients who are investing in aesthetic treatments alongside efforts to manage their weight, this is research worth sharing. It reframes the ingredient conversation from a vague “natural is better” debate into a specific clinical mechanism they can act on.

Nanoplastics and skin cell function

Laura M. Hernandez, Nariman Yousefi, and Nathalie Tufenkji (2017) confirmed the presence of nanoplastics, fragments of polyethylene less than 100 nanometres in size, in commercial facial scrubs. Patients using these products to achieve smoother skin are unaware that the exfoliating particles fall within a size range that has been studied for cellular-level effects.

Ponnusamy Manogaran Gopinath and associates (2022) published a systematic review showing that prolonged exposure to micro- and nanoplastics isolated from cosmetics leads to the down-regulation of cell proliferation in human keratinocytes. Gopinath et al. (2022) note that this cellular disruption has been associated with premature skin ageing. The clinical implication is that some products marketed for skin renewal may have effects that work against the outcomes your patients are seeking from professional treatment.

Where this fits in your patient relationships

Patients trust experienced injectors with clinical observations that go beyond the immediate cosmetic concern. A patient discussing skincare during a consultation is also, in many cases, discussing weight, energy, hormonal symptoms, or general health concerns she may not have raised explicitly with her GP. The “clean beauty” entry point gives you an evidence-based reason to expand the conversation into metabolic and endocrine health without it feeling like an off-topic detour.

Practical advice grounded in the research is straightforward. Patients can audit their daily products for parabens (including methylparaben and butylparaben), eliminate physical exfoliants containing synthetic microbeads on the basis of the Hernandez et al. (2017) findings, and use the science as a framework for making more informed choices going forward. Your role is to flag the research, point them toward the right specialists for any broader endocrine or metabolic concerns that emerge, and ensure that what they apply to their skin at home supports rather than undermines the outcomes of the clinical work you do together.
