The Systems Every Aesthetic Clinic Needs Before ScalingDr Tim Pearce
October 15, 2026

In partnership with JaneApp

Scaling an aesthetic clinic is a process a lot of injectors approach backwards where they assume the answer is simply more patients, more hours, more effort, though what actually determines whether a clinic can grow without falling apart is whether the systems underneath it can carry the extra weight. Dr Tim built his own clinic from £100,000 a year to £3.2 million over eight years alongside his wife Miranda, and almost everything that nearly broke them along the way wasn’t clinical at all, it was operational, which is why he now spends as much time talking about admin and scheduling as he does about anatomy and technique.

Getting the admin sorted before it buries you

janeapp logoWhen Dr Tim did his first aesthetic training back in 2008 he assumed the hard part would be learning where to place the needle, though the real difficulty turned up a few weeks later once he got home and realised nobody had told him what running a clinic actually required day to day. Alcohol wipes, sundries, a reputable place to buy product, registering as a business, insurance, a consent process that actually held up, none of that had been covered on a two day course, and he ended up building a list in an old briefcase one missing item at a time.

What happens for most injectors is that this admin layer keeps expanding quietly in the background while all the attention stays on the clinical side, and by the time a clinic is busy enough to need scaling the admin has usually become the biggest constraint in the whole business. This is one of the areas where a platform like JaneApp earns its place, since patients can complete intake and consent forms online before they arrive and charting sits directly against the appointment rather than in a separate app or a paper file somewhere, which means a huge amount of that background admin simply stops needing a person attached to it.

Building a team that doesn’t depend on one person’s memory

For years Dr Tim was the single point of failure in his own business, where every process, every preference, every bit of pricing logic lived in his head or in Miranda’s, and when they finally started hiring they made a mistake a lot of clinic owners make, which is hiring people they liked rather than people who actually fit the role, without a proper system for those new hires to step into.

The lesson that stuck came later from a colleague describing a plastic surgeon who was still answering his own phone, and the obvious problem there is that a clinic is paying a surgeon’s hourly rate for reception work, which is what happens without a defined team structure sitting underneath the clinical side. Once a role, a schedule and a clear set of expectations exist somewhere other than one person’s memory, a new hire becomes useful in their first week rather than their third month, and this is where JaneApp’s role based access and shared scheduling genuinely helps, giving each team member their own view of the calendar and their own set of permissions so a new starter has a working structure to step into from day one rather than a stack of half explained sticky notes.

Scheduling that stops fighting you

For a long time Dr Tim didn’t take deposits, partly out of a genuine worry that it would put patients off, until one day he drove an hour to a venue and nobody showed up, and after three hours of waiting he went home and deposits were in place within the week. The no-show rate dropped from 15% to 1% almost overnight, and the patients turned out to be far more flexible than he’d feared, it was the business holding itself back rather than the patients.

That single change says something bigger about scheduling generally, which is that most of the friction clinicians imagine patients will object to is actually friction the clinician has built up in their own head. Online booking, automatic deposits and reminders aren’t obstacles for patients, they’re reassurance, and this is exactly the territory JaneApp covers well, letting patients book at any hour without a phone call, taking deposits automatically at the point of booking and sending reminders without anyone needing to remember to do it, which is essentially the deposit lesson minus the year it took to actually act on it.

Keeping things consistent once there’s more than one person doing the work

One of the systems Dr Tim built almost by accident was an automated recall and reminder sequence, having assumed for a long time that patients wouldn’t want to be reminded and that it would come across as nagging. The opposite turned out to be true, patients said they liked it in the same way they appreciate a reminder from their dentist, since it tells them what they need to do to look after themselves and they were probably going to do it anyway.

Consistency becomes a much bigger issue once a clinic has more than one injector, because that’s when small gaps start to appear, a missed follow-up, a forgotten touch-up reminder, a patient who falls through the cracks because nobody was quite sure whose job it was. JaneApp’s automated recall and follow-up sequences handle this quietly in the background, prompting patients when they’re due for their next Botox cycle or checking in after filler regardless of which injector actually treated them, which is more or less the system Dr Tim’s clinic pieced together themselves over several years, just built in from day one instead.

Business infrastructure that lets the clinic run without the owner in the room

The biggest change in how Dr Tim thought about the business came when he accepted that it needed to survive without him physically present, which only happened through documented processes and someone whose job it was to check those processes were actually being followed rather than written down once and ignored. It’s the same principle behind why McDonald’s is identical everywhere in the world, the processes are tight, not because every branch happens to have an exceptional manager improvising well.

Dr Tim and Miranda sold their clinic a few months ago, and it only worked because the business could run without them in it. A clinic owner still doing the books personally at 500 patients a month, or still the only person who understands how a treatment room actually runs day to day, hasn’t built a business yet, they’ve built an extremely demanding job. This is where JaneApp’s reporting genuinely matters, giving real time visibility of revenue, provider utilisation and patient records in one shared system, so decisions get made based on what’s actually happening in the clinic rather than instinct, memory or five different spreadsheets.

Why JaneApp is worth a proper look

Looking back, almost nothing that nearly broke Dr Tim’s clinic was clinical, it was admin that hadn’t been automated, a schedule that hadn’t been protected properly, and processes that lived in people’s heads instead of in a system. JaneApp brings scheduling, charting, payments, patient communication and reporting together into one place, covering most of what took years to build piecemeal, usually after learning the hard way.

Dr Tim genuinely wishes he’d known about JaneApp when he was starting out. For any injector who recognises a no-show problem, a diary full of admin that never quite gets on top of itself, or a growing team that needs more consistency than can be managed manually, it’s worth taking a proper look.
