The jelly roll – that characteristic bulging of the lower lid during smiling – presents an interesting treatment opportunity that varies in desirability across different cultural contexts. In Asia, particularly China and Japan, this feature is often celebrated as a sign of youth and happiness, while in Western aesthetics, patients may seek its reduction.

Understanding the Anatomy

The jelly roll phenomenon occurs through a complex interplay of facial muscles during animation. When patients smile, the orbicularis oculi contracts along with the zygomaticus major and minor muscles, while the lip elevators engage simultaneously.
