Approach necklace lines with dermal filler superficially

neck lines dermal fillerEmbarking on treating his patient’s necklace lines with dermal filler, Dr Tim notes that he always starts by treating the top line around the neck first because they tend to get more difficult as you go lower, particularly if the lines become deeper or thicker, making them more volume heavy and requiring more product.

He prefers to use a cannula for this treatment, tracing along the lines, because there are so many vessels within the neck area, making a cannula a safer option than repeated needle entry.

Dr Tim explains that he wants to see the tip of the canula, pointing upwards, as he moves along each line. This is because placement ought to be superficial, as close to the dermis as possible. Therefore, by purposefully seeing the tip as you trace the line – filling as you slide the cannula with the bevel facing up – you are ensuring the correct placement. He runs a little on either side of the line and not just directly within it. A little bit of trauma from the cannula might also be helpful, alongside the dermal filler product, because it will stimulate increased type four collagen production as a natural wound healing response.

The neck is an area of considerable movement, and lines will persist, depending on patient activity and head position, but the goal is to give the area more stability and improve the aesthetic when in a neutral position.

Massaging the neck after the filler placement is one of the hardest areas to manipulate, warns Dr Tim, because you do not want to compress the area too hard so you can avoid the patient feeling threatened or faint; you must be gentle.

After assessing the primary line filling work with the cannula, Dr Tim turns to some minor tweaks to what remains of the lines using a needle, always staying very superficial when injecting product along the fine lines for the final enhancements.
